Our Healthcare Home program aims to promote healthy lifestyles, encourage self-management of chronic conditions, support healthy use of available health care resources, and coordinate needed services. When working with children and adolescents, our Healthcare Home staff provides education to parents/legal guardians on ways to create healthy lifestyles within the home, information related to their child’s medical condition, medications, and other educational materials.
Healthcare Home takes on a team approach toward effective service delivery. A Primary Care Physician Consultant, Nurse Care Managers, Clerical Support, and Healthcare Home Coordinator/Director make up the Healthcare Home team. This team works closely with psychiatric providers, therapists, caseworkers, primary care service providers, hospitals, pharmacies, and other agencies to provide a holistic approach when addressing the health and wellness of our program participants.

Fees:
Healthcare Home services are paid for through Missouri Medicaid. Other funds may be available to support the cost of such services for eligible individuals who have no other means to pay.
