Community Support/Case Management

Our community support services focus on helping individuals develop skills, access resources and learn to manage symptoms in order to be successful in the living, working, learning, and social environments of their choice.

Community Support for children and adolescents focuses on teaching/modeling developmentally appropriate skills, necessary for positive self-esteem, a sense of identity, positive relationships with family and peers, social competence, and success in school.

Our Community Support Caseworkers will work with our children, adolescents, and families in a variety of settings including, office locations, community settings, individual’s home, and school setting. There are eligibility criteria for this service which would be determined upon evaluation for services. Bollinger, Cape Girardeau, Madison, Perry, and Ste Genevieve counties

Fees:

Community Support/Case management services are paid for through Missouri Medicaid. Other funds may be available to support the cost of such services for eligible individuals who have no other means to pay.
