Our community support services focus on helping individuals develop skills, access resources, and learn to manage symptoms in order to be successful in the living, working, learning, and social environments of their choice. Community Support staff teach, model, and practice skills with persons served in order to increase self-sufficiency and independence. The specific skills and supports are addressed in the client’s individualized treatment plan and based on areas in their life that has been impacted directly or indirectly by their mental health or co-occurring substance use disorder.
Our Community Support Caseworkers will work with our clients in a variety of settings including, office locations, community settings, and the individual’s home. There are eligibility criteria for this service which would be determined upon evaluation for services.
